Yes itās signed and it has a date on it.
Thereās also no option to include any comments/explanation. They accept a document only
Write a response, save as PDF and submit with the LOA. Worth a try ā¦

Well that worked for me. Got the claim dismissed ā¦again
I resubmitted just the LOA and it got accepted so I didnāt need to write a response.
Still having other issues where the same LOA format doesnāt work though..
