Seems fairly neutral to me.
I am not generally in favor of review request inserts, but this one seems to fall within the range of permitted language.

We ourselves have used something similar in much of our Trade Dress for many, many years without problem (we did make a minor change in light of Amazon’s 3Oct`16 Incentivized Review Policy Revision Initiative).

There’s nothing wrong with this. You are not offering anything extra ($/product) and you are not stating “5-Star” or positive review.
Some of the stuff I get from sellers for the stuff we buy from Amazon is incredible. Who has the time to report it all?
