Did they happen to pay for 2 Day or Expedited shipping?
I get the feeling your shipping template may offer a 1-day, 2-day, or Expedited shipping options. (just a quick thought)
My rates are working.
No, it is noted as free shipping and there is no shipping paid shown. I have an identical package that quoted correctly after that one. I’ve refreshed, gone out and back in and am still getting the same result.
Oddly, on Endicia it shows me this, switch (from USPS $7.41) to UPS Ground Saver $7.13.
But when I click on the Switch button, I get $22.48!
Seems like it’s a UPS quote problem. Package is going to Arizona.
The 2nd order I printed is going to New York and the UPS quote was normal.
Did Arizona relocate to another continent? 
My next guess would be that the USPS mail does not deliver to the buyers address in Arizona.
Did you plug the address into the Melissa database to see if the address has mail delivery?

That would not explain the outrageous UPS quote. I was able to purchase a USPS label from Endicia/Stamps, I assume they check the database?
We have had several addresses that do not verify in the USPS system that we can buy USPS Ground Advantage for through Amazon Buy Shipping.
Usually when Amazon does not show USPS options, the estimated delivery on the order can not be met by USPS options. If you have a professional account, we would be checking our shipping templates (especially the one associated with the item where the USPS option is missing).
We have USPS Ground Advantage (less than 1lb) set as our preferred shipping method (and is our only shipping method on our template). We only have Standard shipping enabled (no Economy, no 1 day, no 2 day, no Expedited). All of our orders have their estimated delivery date calculated from USPS GA and we always have USPS GA available to select. If we wanted to ship UPS or FedEx, we could use those options as they are still available.
We would check that SKU to see what shipping template is associated with it.
We would then check that shipping template to make sure it is set up as needed.
I checked this and they are in the database as no USPS mail delivery.
My template assignment is accurate for this item. It is free shipping with a paid expedited option that I need to offer especially during certain seasons.
I already mailed it before you all pitched in to help me here, had to to avoid late shipment. Bought USPS postage from Endicia. I hope they do get mail at the nearest post office and will be picking it up there. If not I will have to refund them.
If they like the product, it could lead to repeat orders and I don’t want to get caught in a situation where I have to lose ~ $14 on every order.

I have not changed my template except to make standard shipping free, yet today, I found a bunch of orders requiring Priority Mail. What happened? I only ever use Ground Shipping.
ETA: Ah, I went in and saw that my shipping template was changed (not by me!). It did not have cheapest option checked. I had that checked before and some bot must have unchecked it.
I’m getting a lot of former GA showing up as Priority today also. Amazon doesn’t think GA will get it there in time. I accepted the risk on one because I was presented with a $9.21 Priority label on an $11.50 sale.
I advise not waiting for the last ship date in the current shipping climate; to avoid being forced into higher cost labels for Buy Shipping on time delivery protection

I have changed some of my sku level handling times today to lengthen the shipping window.
I got this on an order from Thursday or Friday. Less than a pound, usually ships with USPS GA, but this time no USPS options were allowed and UPS Ground Saver also cost over $20.
This is a normal stock item I ship dozens of per week, no idea why this one order was special.
I have been shipping with USPS. It’s cheaper for Ground Advantage. If I’m going to risk being late, might as well pay less. There is no way I’m paying Priority when 1) it costs more than the product and 2) USPS says it will be there on time. I’m gambling, but if some arrive late and they want a refund, so be it.

Possible. But that doesn’t explain why the UPS Ground Saver was over $20 for a 10oz 6"X4"X3" package.
If that’s the case, I wouldn’t have been able to create a label on the USPS website.
I was referring to why @HobbesIsMyTiger was not presented a USPS option on Amazon buy shipping. At times when I’m not presented USPS as an option, I go to Pirateship which will let you purchase whether it was validated or not by USPS system.
I have a USPS account that I use for shipping Walmart, Ebay, etc. That account offered normal shipping options. Only Amazon was giving me no USPS and very expensive UPS options.
I encountered this today on 5 orders. If I view all options, USPS is there further down the list, but greyed out with Late Delivery Risk in red. I can still choose and purchase it, knowing that it will be late and not covered by the Buy Shipping Guarantee, and will be a metric hit.
It’s not for sure that the package will be late, it’s only it “maybe” late.
I have purchased Amazon shipping labels (USPS GA) with “Late delivery risk”, and only one package was late by a day, all the rest were all delivered couple days before the deliver by date.
